The following outlines the specific procedures required to obtain a state amusement ride operating permit:

  • Complete and submit an Amusement Ride Operating Permit application form to the Missouri Division of Fire Safety for each ride.
(Download Amusement Operating Permit Application now)
  • Submit Amusement Ride Inspection Report signed by a state approved qualified amusement ride safety inspector for each amusement ride verifying that such ride(s) meets Missouri's safety inspection requirements.  Permits are valid for 12 months from date of inspection and must be renewed annually.   Nondestructive testing documentation, if applicable to that ride, shall be submitted with the inspection report.

  • Provide documentation to comply with insurance requirements outlined in RSMo 316.210.  Click here for complete insurance requirements.
  • Each amusement ride owner/company shall file an itinerary (available here) with the Division of Fire Safety no less than fifteen (15) days before event dates.
  • Effective April 30, 2005 Amusement Ride Permit Fees will be $30.00 per ride per year.  Check or money order made payable to the Missouri Division of Fire Safety will be accepted.  Cash will not be accepted.

Upon receipt of application, approved inspection, proof of insurance and permit fee a Missouri Amusement Ride Operating Permit will be issued.  The Missouri Amusement Ride Operating Permit consists of two parts:  a permit form which must be stored at the location of the amusement ride, and a permit decal which must be adhered to the control panel of the respective amusement ride.

Please allow thirty (30) days for processing of paperwork and permit issuance.  Based on the number of requests received in 2004 to issue a "permit waiver letter" due to the owner/company failing to submit necessary documentation in a timely manner may not be an option in 2005.  If such documentation is not submitted within two weeks prior to the scheduled event an operating permit may not be issued for that event.  The Missouri Amusement Ride Operating Permit decal must be displayed on the ride before the ride is to operate.
